OVARIANA app terms — draft
Draft for owner and legal review. This text is not an approved agreement. No acceptance is collected on this page.
1. Provider and scope
OVARIANA is provided by MEDAI sp. z o.o., ul. Prof. dr Mieczysława Michałowicza 12, 43-300 Bielsko-Biała, Poland (KRS 0000827308, NIP 5472217169, REGON 385526755). These draft terms concern the patient web application at app.ovariana.app. The information website at ovariana.app is a separate service. Contact the provider through the contact page linked below.
2. What the app offers
The app lets you record your own observations, review your journal and prepare a non-diagnostic summary for a healthcare conversation. Available functions may change during Preview. No subscription or paid report purchase is offered under this draft; any future paid service requires separate terms and clear price information before ordering.
3. Not medical care
The app does not diagnose, predict disease, recommend treatment, monitor emergencies or replace professional medical advice. Do not delay seeking care in order to complete an entry. In an urgent situation, contact local emergency services or a healthcare professional.
4. Account and access
Use an account you control and protect your sign-in details. Do not access someone else's account or submit another person's health information without a valid right to do so. If access is lost or you suspect misuse, contact the provider. The app may temporarily limit access to protect accounts, investigate abuse or perform essential maintenance; mandatory consumer rights remain unaffected.
5. Your entries and corrections
You choose what to record. A missing entry is not a statement that a symptom is absent. Check the accuracy of dates and descriptions before sharing a report. The app retains source and correction history where needed to present an accurate record. Your personal notes are not translated automatically.
6. Consent and personal data
Consent to process health data is requested separately after account creation. Withdrawing it in Settings pauses access to the journal and ends active report-sharing grants; it does not automatically delete saved data or recall a report already viewed or saved by a clinician. This draft is not a privacy notice for the app. App-specific privacy information, including rights, retention, processors and transfers, must be made available before release. The ovariana.app website privacy policy explicitly does not cover app accounts.
7. Reports and sharing
You decide whether to generate a report and whether to grant a clinician access where that feature is available. A grant can be ended, but a copy already seen or saved by its recipient cannot be recalled. The report reflects the selected interval and recorded sources; it is not a diagnosis or a clinical decision.
8. Acceptable use and intellectual property
You retain your rights to content you enter. MEDAI may process it only to provide the functions you choose and in accordance with the applicable privacy information and permissions. Software, layout and OVARIANA branding belong to their respective rightsholders. Do not interfere with the service, bypass access controls or misuse other users' data.
9. Changes and availability
A Preview function may be revised or unavailable. Material changes to the final terms or the service must be communicated through an appropriate channel before they take effect; where a new acceptance is required, it will be requested separately. These draft terms do not authorise processing beyond the current consent.
10. Your rights and contact
Nothing here excludes statutory consumer rights or liability that cannot lawfully be excluded. Mandatory protections of the country where you habitually reside remain applicable. For questions, complaints or accessibility needs, use the provider contact page. The final terms must specify the complaints process, contractual formation, minimum age and any withdrawal rights after legal review.
